Premier Sports Medicine is not a medical clinic or physician practice. Athletic training services are provided by licensed Athletic Trainers in accordance with applicable state licensing laws and professional standards, and under physician supervision where required by law.

Use of this Site does not establish a provider–patient, clinician–patient, or any other professional healthcare relationship.

4. HIPAA & Protected Health Information

This Site is not intended for the collection or transmission of Protected Health Information (PHI) as defined by the Health Insurance Portability and Accountability Act of 1996 (HIPAA). Do not submit medical diagnoses, treatment details, insurance information, or other sensitive health data through the Site unless specifically instructed to do so through a secure, designated method.
